Informed Consent

I understand that Monette Mudie is a Licensed Spiritual Healer and an Ordained Minister in the Order of Melchizedek. Monette is also a Certified Reiki Master Teacher, trained in the SomaEnergetics Energy/Body Tuners, Sound Healing Therapy, Beyond Quantum Healing, and Emotional Freedom Techniques. Natural Therapies Certification and the Federation of Spiritual Healer Licensing Boards also approve her. Monette uses intuition, aromatherapy, and all modalities trained to help kick in my body’s natural healing processes for overall well-being. I also understand that energy healing and sound healing can help me to relax, reduce my stress and anxiety, and manage any pain. I further understand that she also offers spiritual life coaching that will teach me how to use specific tools and techniques to empower me to improve the quality of my life.

I understand that I am responsible for my health, healing, and well-being. Therefore, I agree to use the services provided by Monette Mudie to learn how to improve the quality of my life. I also understand that natural healing is not a substitute for medical care, and I am responsible for maintaining a relationship with a medical doctor if I so desire. I understand that Monette Mudie is not trained to diagnose illness, make pharmaceutical drugs or surgery recommendations, or handle medical emergencies.

I understand that Monette Mudie will keep all information she learns about me in the strictest confidence unless I release her in writing or as required by law. I have the right to waive this confidentiality agreement in whole or part at any time.

I understand that if I have or think I have a medical concern, condition, disease, disorder, issue, or symptoms, Monette Mudie will help me reduce any related stress and refer me to a licensed chiropractic, medical, or osteopathic physician for further assistance.

I also understand that if I have or think I have a psychological or emotional concern, condition, disease, disorder, issue, or symptoms, Monette Mudie will help me reduce any related stress and refer me to a licensed counselor, psychologist, or psychiatrist for further assistance.

I acknowledge that I have read and understand this form. I agree to allow Monette Mudie to offer a safe space to help me enable and accept divine healing using the natural healing techniques and modalities listed herein.
